Insurance/Billing

We are an approved provider for most major insurance carriers and health plans.  A list of these insurance plans with our provider number, if applicable, can be viewed by clicking here.  If your insurance company is not listed, call us for further information.  What your insurance plan covers depends on your specific insurance plan.

 

Preauthorization

There are many different insurance carriers with each company having a variety of different plans.  Some insurance companies and/or insurance plans have no preauthorization requirements and some may require preauthorization before an examination can be performed.

 

If your insurance plan requires preauthorization, every effort will be made by our Insurance Department to assist you and your physician in obtaining the necessary authorization for your procedure.  If, at the time of your examination, you have been unable to obtain a required preauthorization, you may have to consider rescheduling your appointment until the authorization can be obtained from your insurance company.

 

Claim Submission

We will bill your insurance company on your behalf under the benefit coverage you specify including workers’ compensation, auto insurance and personal injury.  Ultimately, you are responsible for payment of the account including any balance not covered by insurance.  You will receive statements informing you of the status of your claim.

 

We encourage you to stay involved with your insurance company to ensure your claim is paid.  Any amount not paid timely by your insurance company may become your responsibility.  Once we hear from your insurance company regarding your claim we will send you a balance statement for any amount of coinsurance, copayments and/or deductible you owe.  

 

Self Pay

If you do not have medical insurance and you are concerned about paying for your procedure, call us to receive an estimate of the cost of your procedure and to discuss payment options.  Special cash discounts and interest free payment plans are available for self-pay patients.

 

Payments

Depending on your insurance plan coverage, you may be responsible for the payment of coinsurance, copayments and/or deductibles.  We realize that some individuals may not be able to pay their entire balance in full.  If full payment cannot be made, an interest free payment plan can be arranged.  To make payment arrangements please contact our Billing Department.

 

Attorney Letter of Protection

If your procedure is ordered because of a personal injury for which you have an attorney handling your case, we do accept an attorney letter of protection for payment of your account.
