It is important to realize that as a patient at
our facility you have both rights and responsibilities. This summary
of patients’ rights and responsibilities describes how you can
participate in receiving quality care.
PATIENT RIGHTS AND RESPONSIBILITIES
Jacksonville Beaches Medical Imaging believes
that each patient is entitled to certain rights and is committed to
the responsibility to assure that these rights are preserved for its
patients. As part of that commitment, care shall be provided
impartially to all patients, regardless of race, creed, gender,
national origin, religion, sexual orientation, or source of payment.
Patient Rights

treated with respect, consideration and dignity at all times.

right to personal safety and to receive care in a safe setting.

right to be free from all forms of abuse and harassment.

right to receive information about procedures including
associated risks.

right to consent to, or refuse, any and all treatment.

right to know the identity and professional status of all
individuals providing care.

right to information about services and any related costs.

right to receive an itemized explanation of bill for services
rendered.

right to confidentiality of all medical records.

right to receive a copy of the facility’s Notice of Privacy
Practices related to HIPAA.

right to express a concern or complaint to the Administrator or
staff.
Patient Responsibilities

The responsibility to provide accurate
information about present and past health history.

responsibility to ask questions if directions or procedures are
not clearly understood.

responsibility to treat all facility personnel respectfully and
courteously.

responsibility to understand one’s health insurance coverage
benefits and limitations.

responsibility to provide accurate, up-to-date insurance
information.

responsibility for assuring that financial obligations for
services rendered are met.

responsibility to express opinions, concerns or complaint in a
constructive and appropriate manner.
